effect of intravenous methypredinsolone pulse on the infantile spasm.

objective: infantile spasm is an age dependent ant myoclonic seizure which occurs in 25 per 100,000 live births. we aimed to investigate effect of intravenous methypredinsolone pulse on the infantile spasm in patients hospitalized in 17 shahrivar hospital, rasht. materials and methods: this is an observational case series which was conducted on 20 infants with infantile spasms hospitalized in 17 shahrivar hospital during 2011-2014. infants aged less than 1 year and more  than 1 month with any etiology of spasm were included. primary outcome was the control of spasm and the second outcome was the omission of hypes arythmia in infants follow up. remission was indicated if the caregivers mentioned no spasm or >50 % decrease regarding drug initiation for at least 5 consecutive days and the eeg during sleep period noted the omission of hypes arythmia. results: mean rapid remission was noted as  4.4167±1.50504 days.  at the follow up eeg, 13 patients(65%) were normal and 7 (35%)abnormal cases were noted. no complication was noted in most of the patients. (13 cases) however, 5 weight gain, 1 asymptomatic glucosuria and 1 irritability were noted in patients with infantile spasm. conclusion:  regarding similar effects of pulse methyl predinsolone in comparison with acth, it seems that methyl predinsolone is a preferable drug regarding easy administering, low cost  and its  accessibility.
